Cheryl Middleton, FNP-BC, is a board-certified Family Nurse Practitioner who has provided dedicated healthcare to Chester County and its surrounding areas for more than 26 years. Cheryl leverages a diverse medical background and vast professional experience, focusing heavily on chronic disease management and preventative health care. Committed to accessible medicine, she currently offers specialized house call services to meet the vital primary care needs of homebound and disabled patients within our community.

Traci Hill, FNP-BC, is a board-certified Family Nurse Practitioner bringing more than 30 years of extensive critical care, radiology, and family practice nursing experience to our clinic. Since 2012, Traci has practiced exclusively as a primary care provider within the rural health community, giving her a deep understanding of local healthcare needs. She is completely devoted to providing high-quality, individualized medical care tailored to the unique health journey of each patient she serves.

Nicole Crozier, FNP-BC, is a board-certified Family Nurse Practitioner backed by more than 24 years of dedicated nursing experience. Nicole’s diverse background includes specialized clinical work in urgent care, neonatal intensive care (NICU), and school health settings. At our clinic, she provides comprehensive general and preventative care, performs routine annual wellness physicals, and is fully certified to conduct Department of Transportation (DOT) physicals to keep our community safe and healthy.


Kase Hamm, FNP-BC, is a board-certified Family Nurse Practitioner who brings a highly skilled and compassionate approach to our clinical team. Kase possesses an extensive medical background with specialized experience in both the coronary intensive care unit and hospice care. This unique combination of critical care and palliative training equips her to expertly address the complex, evolving health needs of individuals across their entire lifespan. She remains deeply dedicated to delivering high-quality, continuous patient care and advancing her professional development in family medicine.
Josh Fortner, LMSW, is a Licensed Master Social Worker dedicated to providing compassionate counseling services to individuals of all ages. Josh holds a degree in education from Central Baptist College in Conway, Arkansas, and earned his social work degree from Union University. Combining his diverse background in education and behavioral health, Josh utilizes effective therapeutic interventions to support his clients through life's challenges, aiming to make a lasting, positive impact on their mental health and overall well-being.


Cameron Burks serves as the Practice Administrator at West Tennessee Primary Care, LLC, where she plans, coordinates, directs, and supervises all elements of our clinic. Cameron drives our mission forward by developing long-term business strategies, managing the budget, and overseeing billing and collections. From designing innovative patient services and implementing employee work policies to assisting with day-to-day operations, her comprehensive leadership ensures a high-quality, seamless healthcare environment for our staff and patients alike.
Ashtyn Young serves as a medical receptionist at West Tennessee Primary Care, LLC, where she coordinates the daily administration of our providers, staff, visitors, and patients. Ashtyn ensures our facility runs smoothly by scheduling appointments, organizing documentation, and accurately maintaining patient records and accounts. As a welcoming first point of contact, she expertly answers and refers inquiries while remaining fully trained to respond calmly and efficiently during medical emergencies.


Sherry Newman is our dedicated medical biller and a cornerstone of our facility, having served our practice for more than 31 years. Sherry manages the entire financial side of patient care by preparing, reviewing, and transmitting medical claims. She calculates and collects payments for procedures, designs manageable payment plans, creates invoices, and keeps patient financial data up to date, ensuring a smooth and supportive billing experience for everyone.
Alesha Wallis, CMA, is a Certified Medical Assistant bringing more than 21 years of invaluable medical experience to our practice. Alesha plays a critical role in supporting our patient community by arranging diagnostic tests, handling specialist referrals, and assisting directly with clinical patient care. She also serves as a trusted point of contact, answering telephone inquiries and delivering vital health education to patients and their families.


Tyler Meek is a versatile patient care technician with an extensive clinical background spanning pediatrics, family practice, and home care. Tyler handles essential patient intakes, monitors vital signs, performs clinical tests, and administers treatments, medications, and vaccinations. In addition to delivering high-quality, direct patient care, he plays a key operational role by coordinating both home and facility visits to ensure seamless, accessible healthcare for our community.
Lori Presnull is a highly skilled clinical assistant with more than 20 years of dedicated experience in primary care. Lori manages comprehensive patient intakes, monitors vital signs, performs routine tests, and administers treatments, medications, and vaccinations. Expertly trained to intervene during medical emergencies, she also provides vital patient education and ensures all clinical equipment is meticulously maintained to provide a safe, high-quality care environment.


Maria Lewis serves as a vital clinical assistant in our practice, blending hands-on care with exceptional administrative expertise. Maria manages essential patient intake by recording medical histories, updating data, and monitoring vital signs. She also performs clinical tests, processes referrals, and provides valuable patient education. With nearly 10 years of prior experience as a medical receptionist, Maria brings a uniquely well-rounded understanding of patient care from the front desk to the exam room.
Kealeia Scott brings over 15 years of dedicated medical experience to our clinic. Her diverse background spans geriatric care in nursing homes, specialized home health for pediatric and geriatric patients with unique needs, and primary care. Expertly managing chronic conditions while overseeing our day-to-day facility operations, Kealeia seamlessly balances high-quality clinical care with administrative excellence to support our patients and staff every day.


Emily Myers, ASCP, is a board-certified phlebotomist with 19 years of extensive clinical experience. Employed by Labcorp, Emily works directly within our facility to provide dedicated, high-quality lab services. She is highly skilled in all clinical settings, ensuring that every patient sample is accurately labeled, carefully handled, and safely transported for testing. Emily’s decades of expertise bring precise, reliable care directly to our patients.
