Our goals, ambitions, and inspirations have come together to form a truly amazing primary care clinic in Henderson, Tennessee. Mission
West Tennessee Primary Care, LLC has a mission to provide individualized holistic health care for all ages by proactively assessing the needs of each individual, as well as providing appropriate preventive care and mental health services. Our focus is to develop a personalized plan of care for each patient while implementing a team-based approach to efficiently meet patient needs.
Services
West Tennessee Primary Care, LLC provides continuous, comprehensive, and individualized health care services for the entire family. We strive to integrate biological, clinical, and behavioral sciences by offering holistic care while engaging patients and their families in decision-making with each aspect of care. Specific services include individualized and group counseling, home visits for physically disabled and homebound individuals, medication management, sick visits, wellness examinations for children and adults, pre-participation sport physicals, Department of Transportation certifications, as well as management of chronic health conditions, physical injuries, and more.
Our Team
Cheryl Middleton is a board certified Family Nurse Practitioner providing care to Chester county and surrounding areas for more than 25 years. She has a diverse medical background and vast professional experience with focus on chronic disease management and preventive health care. She is currently offering house call services to meet the primary care needs of homebound and disabled patients in the community.
Traci Hill is a board certified Family Nurse Practitioner with more than 30 years of critical care, radiology, and family practice nursing experience. She has practiced exclusively as a primary care provider within the rural health community. Traci is devoted to provide high quality, individualized, and holistic care to each patient.
Nicole Crozier is a board certified Family Nurse Practitioner who has more than 23 years of health care experience including neonatal intensive care. Nicole provides general and preventative care, conducts wellness examinations, treats illnesses, orders lab tests, and prescribes medication for children and adults.
Brittany Naylor is a Licensed Clinical Social Worker who has nearly 10 years of counseling experience with various age groups. She has expertise skills in coordinating and implementing interventions for children and adults suffering with depression, anxiety, substance abuse, and other mental health concerns. Brittany provides individualized sessions in person or by implementing video technology.
Cameron Burks is Practice Administrator at West Tennessee Primary Care, LLC. Cameron plans, coordinates, directs, and supervises various elements of the clinic. She also oversees billing and collections, designs patient services, and implements work policies for employees. Cameron also develops business strategies, manages budget, and assists with day-to-day operations.
Ashtyn Young is a medical receptionist at West Tennessee Primary Care, LLC. She coordinates the daily administration of providers, staff, visitors, and patients within the facility. Additionally, Ashtyn schedules appointments, coordinates documentation, answers and refers inquiries, maintains records and accounts, and responds to emergencies.
Sherry Newman is a medical biller and has been employed at this facility for more than 30 years. Sherry is in charge of preparing, reviewing, and transmitting claims, calculating and collecting payments for medical procedures and services. Developing payment plans, creating invoices, and updating patient data are also responsibilities.
Alesha Wallis is a medical assistant with more than 20 years of medical experience. She arranges diagnostic tests/procedures, specialist referrals, and assists in patient care. Additionally, Alesha answers telephone inquiries from patients and provides education to patients and their families.
Tyler Meek is a medical assistant with clinical experience including pediatrics, family practice, and home visits. Tyler conducts patient intakes, monitors vital signs,
administers treatments, medications and vaccinations, performs tests, and provides direct patient care. He is currently responsible for coordinating home and facility visits.
Lori Presnull is a clinical assistant who has more than 18 years of experience working in primary care. Lori conducts patient intakes, administers treatments, medications and vaccinations, monitors vital signs, performs tests, intervenes during medical emergencies, provides patient education, and maintains equipment.
Maria Lewis is a clinical assistant who records medical history, obtains and updates patient data, completes pertinent documentation, and offers patient education. Additionally, Maria monitors vital signs, performs tests, and completes referrals. Maria also has almost 10 years of experience as a medical receptionist.
